The length and pattern of perimenopause are individual, so one woman's experience may look quite different from another's.

Why Hot Flushes Can Occur

Hot flushes are among the better-known changes associated with menopause. They may involve a sudden sensation of warmth, particularly around the face, neck, or upper body, sometimes accompanied by sweating.

Changes in estrogen are thought to influence the brain systems involved in regulating body temperature. During the menopausal transition, the body may become more sensitive to relatively small changes in internal temperature.

Poor sleep can then influence daytime energy, concentration, appetite, and emotional wellbeing. This means that what initially appears to be one menopause-related issue may overlap with several areas of daily life.

Mood and Concentration Changes

Some women notice greater irritability, changes in mood, anxiety, or difficulty concentrating during the menopause transition.

Hormonal fluctuations may contribute, but emotional wellbeing during midlife is rarely explained by hormones alone. Sleep quality, work demands, relationships, caregiving responsibilities, physical health, and personal circumstances can all influence mood.

Occasional forgetfulness or difficulty concentrating is also sometimes described informally as “brain fog.” Experiences vary, and persistent or significant cognitive or emotional changes deserve individual attention rather than automatically being attributed to menopause.

Maintaining muscle through regular activity and strength-based exercise can therefore be valuable during and after the menopause transition.

Vaginal and Urinary Changes

Lower estrogen levels can affect tissues in the vaginal and urinary areas. Some women may experience vaginal dryness, irritation, discomfort during sexual activity, or changes involving urinary comfort.

These changes can sometimes develop gradually and may continue after other menopause symptoms have become less noticeable.
